About Us

The Department of War (DoW) Forms Management Program is administered by the Directives Division (DD), Executive Services Directorate, Washington Headquarters Services. In coordination with the DoW, CIO, the DD establishes policies and procedures for the DoW Forms Management Program, including responsibilities for reviewing and approving form requirements. DoW forms satisfy a valid need; essential to accomplish a mission and necessary for the efficient and economical operation of the DoW.

This web site provides the “Official” edition of Department of War (DW) and Secretary of Defense (SD) forms. The DoW Forms Management Program web site serves as the single source of information for forms belonging to and used by the Department of Defense in accordance with “Web Site Administrative Policies and Procedures.”

Furthermore, the DoW Forms Management Program serves as the single source of information for blank forms belonging to and used within the Department of War.  This means that we are responsible for making sure that all Military Services and DoW components remain compliant with established form policies, and that all approved forms are current and readily accessible.  We do not receive or process completed forms.  For status of a completed form, please contact the appropriate component and/or the Office of Primary Responsibility (OPR) in order to address your concerns.

The following documents are integral to the DoW Forms Management program.

  • DoW Instruction 7750.07, “DoW Forms Management Program”
    • This instruction establishes policies, assigns responsibilities, and provides procedures governing the DoW Forms Management Program.
  • DoW Manual 7750.08, “DoW Forms Management Program (FMP) Procedures”
    • This Manual is issued under the authority of DoW Instruction 7750.07 (Reference (a)). It implements policy, assigns responsibilities, and provides specific procedures for management of the DoW Forms Management Program.
